Premises: The 2023 intersociety guidelines on the management of diabetic or hyperglycemic patients admitted to a non-critical setting reiterated that it is desirable to adopt an organizational model that includes the intervention of personnel with diabetological expertise where available, and that a structured discharge plan is more cost-effective than a discharge method that does not include one. In 2023, the Internal Medicine department at Giglio Hospital in Cefalù created an outpatient service for the care and follow-up of diabetic patients after hospitalization, with direct access and appointments within a month of discharge.
Materials and Methods: Between February 2023 and January 2024, 120 subjects were admitted (58% discharged from medicine, 39% from cardiology-rehabilitation, 3% from other departments), and the changes in HbA1c, the use of insulin, GLP1RA, or SGLT2i during hospitalization and follow-up (1/3/6 months) were retrospectively evaluated in 100 of them.
Results: Compared to background antidiabetic therapy, the use of innovative molecules (GLP1RAs or SGLT2is) increased statistically significantly during hospitalization, remained constant in subsequent check-ups, and contributed to a statistically significant reduction in mean HbA1c and insulin requirements. The readmission rate was <2%.
Conclusions: The PDTA for people with diabetes admitted to Cefalù Hospital represents an effective tool for improving diabetes outcomes and promoting adherence to a chronic care plan that is more in line with current Italian guidelines.
