Preface: Long Covid Syndrome: signs and symptoms that persist after an acute SARS-CoV-2 infection for at least 4 weeks, with even severe events occurring remotely. There are no pathognomonic diagnostic criteria/biomarkers/instrumental markers.
Materials and Methods: Opening of a specific outpatient clinic at San Bortolo Hospital, Vicenza; 4 weekly visits; a dedicated internist. Patients referred by general practitioners, emergency services, occupational health physicians, and other specialists.
Results: May 2021 - December 2023. 313 patients: 108 (35%) males, 205 (65%) females, average age 51 years. 291 first visits and 59 follow-up appointments. Required further information: Cardiology (14), Neurology (14), Physical Therapy (4), Rheumatology (6), Psychiatry (3), Pulmonology (10); scheduled hospital admissions (4). Signs/Symptoms: asthenia, pain, reduced exercise tolerance, insomnia, anxiety/mood disorders, tachycardia, cough, low-grade fever, taste/smell alterations. Diagnosis of non-Covid-related organic pathologies: Familial Mediterranean fever (1); bacterial endocarditis (1); subacute bacterial osteomyelitis (1); heart failure (1); eosinophilic granulomatosis with polyangiitis (1); fibromyalgia (3). Prescribed therapies: Supplements with carnitine, alpha-lipoic acid, PEA, coenzyme Q; anxiolytics, hypnotics; neuromodulatory analgesics; antidepressants; beta-blockers.
Conclusions: Long Covid syndrome represents a long-term multisystem complication of SARS-CoV-2 infection, with a severe impact on quality of life. Differential diagnosis with organic issues is crucial, using targeted investigations, with a dedicated and trained outpatient clinic and specialist.
