Study Background and Purpose: The Department of Nursing and Midwifery Professions of the ASL Toscana Sudest Healthcare Company is implementing a Professional Practice Model rooted in Relationship-Based Care (M. Koloroutis). This vision lays the theoretical foundation and practical infrastructure for driving cultural change aimed at providing empathetic care and personalized nursing assistance.
Materials and Methods: Implementation of the primary nursing care model in medical areas, along with the GNNN, a care process involving personalized nursing care planning thru nursing assessment using M. Gordon's model, international languages with nursing diagnoses - NANDA International, care outcomes with Nursing Outcome Classification, and care interventions with Nursing Intervention Classification. Using the care outcomes monitoring dashboard to verify the improvement in the patient's health.
Results: By the end of 2023, the primary nursing and GNNN organizational model had been implemented in 80% of the company's Medical Areas. It was possible to analyze the application of nursing assessment and targeted evaluation forms, nursing diagnoses, objectives, and interventions.
Conclusions: The use of organizational models and care planning with the GNNN model makes the outcomes sensitive to the nursing care provided traceable and, above all, measurable.
